Which tenses do we use?
PAST TENSES

She had eaten the apple.--> What had she eaten?
She had been eating the apple.--> What had she been eating?
She used to eat the apple.--> What did she use to eat?
She would eat the apple.--> What would she eat?
Important pointers:
Normally , the time expressions, the adverbial clauses , etc. are going to indicate which event was first and which other was 2nd. However, using past perfect facilitates everything.
I had breakfast before I went to work.
before I went to work, I had breakfast .
I went to work when I had breakfast.
when I had breakfast, I went to work .
I went to work after I had breakfast.
I went to work as soon as I had breakfast.
I had had breakfast before I went to work.
I went to work when I had had breakfast.
I went to work after I had had breakfast.
I went to work as soon as I had had breakfast.
I had been having breakfast before I went to work.
used to = would (except location, state, and possession)
I used to/would study in Saint Rose School.
I used to/would live in Trujillo.
I used to/would be shy.
I used to/would have a Rolex.
Factual conditional in the past:
If I was on vacation, I would/used to visit my grandfather.
Imaginary conditional in the present
If I were on vacation, I would go to the beach.
He has lived in Lima for 20 years.
He has lived in Lima since 1996 .
He has lived in Lima since 20 years ago. <-- Perfect!
He has lived in Lima. (no time expression --> it is past)
He ___lived_____ in Lima in 1996 / when I was a child.
He __has lived__ in Lima since 1996 /since I was a child.
He __has lived___ in Lima so far = until this moment.
He ___lived/ has lived_____ in Lima .
